Dietitians at Home services include a Registered Dietitian who works with diabetic patients in the comfort of their own home. Our clinical program focuses on individualized patient plans which encourage incremental behavioral changes to improve patient outcomes. Our dietitians build long-term relationships with our patients and participate on an ongoing basis in the patients care coordination team. We follow our patients over an extended period of time, providing Medical Nutritional Counseling that drives on-going improvement in our patients nutritional understanding, dietary behavioral changes and clinical outcomes.
Job Description
Dietitians at Home provides Medical Nutritional Therapy to patients with Diabetes and Chronic Kidney Disease (not undergoing dialysis) in the comfort of the patient’s home. It was started to provide resources to an under-served population that is often overlooked by traditional healthcare systems, Dietitians at Home works to prevent the development of and delay progression of chronic illnesses.
We are currently seeking a full time Office Support Professional to be responsible for all daily functions of the office.
The daily responsibilities include:
* Processing new patient referrals
* Distributing all faxes to the appropriate people
* Answering the phone and either handling the calls or transferring the
call to the appropriate party
* Preparing patient schedules for the dietitians
* Faxing notes for patient visits to Nursing Agency’s and Doctors
offices
* Faxing and following up on Certificates of Medical Necessity
* Putting together patient charts and paperwork
* Providing in office support to the Dietitians in the field
* Implement orders from Dietitians
* Working with sister companies to provide other company services to
our patients
Requirements:
Bilingual in Spanish, preferred
Bachelor Degree, preffered
At least one years relevant work experience
Proficient with Microsoft Office
Advanced project management and organizational skill
Excellent written and verbal communication skills
Proactive problem solver
